What this actually involves
Iowa asks for more paperwork than most families expect: an immunization certificate, a dental screening certificate, a vision screening for kindergarten and third grade, and a separate pre-participation physical evaluation for school sports. All of it has different timing and different signatures.
We do all of it at one appointment, and we prefer to do it inside the annual well-child visit so your child is not sitting through two exams in one summer. Forms go back through the portal, usually the same day.

Iowa Certificate of Immunization
Printed and signed from the IRIS record, in the format Iowa schools and licensed childcare centers accept.
Pre-participation physical evaluation
The IHSAA and IGHSAU form, following the PPE Monograph: full cardiovascular history, personal and family sudden-death screening, musculoskeletal screen and concussion history.
Vision and dental certificates
Vision screening for kindergarten and third grade entry, and we tell you which dental screening certificate deadline applies to your child's grade.
Medication at school
Authorization forms for inhalers, epinephrine auto-injectors, insulin and daily medication, including self-carry authorization where it is appropriate.
504 and IEP input
Written medical input for asthma, diabetes, food allergy, ADHD and seizure plans, sent directly to the school nurse when you sign the release.
Camp and travel
Overnight camp forms, YMCA forms and international travel documentation, with a note on what we can and cannot sign.

How it runs
- 01Upload the form
Send the blank form through the portal before the visit. It is the single biggest thing that speeds this up.
- 02Book in spring
May and June appointments are far easier to get than the week before school starts.
- 03At the visit
Full exam plus the specific screening each form requires. Your child comes into the room in shorts and a t-shirt for the musculoskeletal screen.
- 04Turnaround
Completed forms return through the portal within two business days, or the same day if the form is in hand at the visit.

All FAQsIowa requires a dental screening certificate at kindergarten entry and again at ninth grade, and the screening window for each is defined by the grade. We tell you at the visit which window your child is in and what counts as an acceptable screener.
No. A sports physical is a narrower screen aimed at safe participation. A well-child visit includes it and adds growth, vision, hearing, blood pressure, mental health screening and vaccines. If you only do one, do the well visit.
We can, but we will want a real visit first. Signing a clearance form for a child we have never examined is not something we are willing to do.
Iowa high school athletics accepts a physical for the school year it covers. We date the form so it carries your athlete through the season they are actually playing.
