Iowa Certificate of Immunization
Every child entering an Iowa school or licensed childcare

- Child's full legal name and date of birth
- Any out-of-state records you hold
- Nothing else, we pull the rest from IRIS
Dental screening certificate
Kindergarten entry and ninth grade entry

- Name of the dental home if your child has one
- Date of the last dental exam
Vision screening certificate
Kindergarten and third grade

The order to do it in
Front-load the summer. Every practice in Polk County is full in the third week of August.
- April to JuneBook the annual well visit
One visit produces every form your child needs for the year. In May we can usually see you inside a week.
- JulySports clearance and inhalers
IHSAA and IGHSAU forms before the first practice, plus a pre-season asthma review before ragweed starts.
- AugustCertificates and medication authorizations
Immunization certificate printed, dental and vision certificates confirmed, school medication forms signed for the new year.
- Any time504 and IEP meetings
Tell us the meeting date at least a week ahead. Written medical input takes five business days.
Upload the blank form before the visit
This is the single thing that changes a two-day turnaround into a same-day one. A form in our hands during the appointment is signed in the room. A form handed over at the desk on the way out has to be scanned, matched to a chart and routed to a clinician who is now seeing someone else.

Questions about forms
Same day if the blank form is uploaded to the portal before the visit. Two business days for forms handed to the front desk afterwards, and five business days for 504 and IEP medical input, which is genuinely longer to write. Sample service standards for this demonstration site.
No separate form fee for established patients. Forms completed inside a well-child visit are covered as part of that preventive visit by most plans.
No. Signing a participation clearance for a child we have never examined is not something we are willing to do, and we do not backdate physicals.
A medication authorization signed for this school year, plus a separate self-carry authorization. We will also talk with you about whether your child is ready to carry it, which is a real conversation rather than a formality.
Book a visit. A sports physical covers the school year it is dated for, and Iowa schools hold to that. We will work out what is possible in the time you have.
