Information we collect

We only collect what we need to respond to you and provide our services.

• Contact details you give us, such as your name, email address, phone number and address.
• Details about the service you are requesting, entered in quote, booking or contact forms.
• Technical data such as browser type, device and pages visited, collected through cookies you allow.
• Records of our communications with you, including calls, emails and messages.

How we use your information

We use personal information to:

• Respond to enquiries and prepare quotes or appointments.
• Deliver, schedule and follow up on the services you request.
• Send service reminders and, where you have agreed, occasional offers.
• Improve our website and meet our legal and regulatory obligations.

Sharing

We do not sell your personal information. We share it only with trusted providers who help us run our business, such as scheduling, payment and email platforms, and only under agreements that require them to protect it. We may disclose information where the law requires it.

HIPAA Notice of Privacy Practices

Little Sprouts Pediatrics is a covered entity under the Health Insurance Portability and Accountability Act. This notice describes how medical information about your child may be used and disclosed, and how you can get access to that information. A printed copy is given to every family at the first visit and is available at any front desk.

We are required by law to maintain the privacy of protected health information, to give you this notice of our legal duties and privacy practices, and to notify you following a breach of unsecured protected health information.

• Treatment: we use and share health information to provide care, including with specialists we refer your child to, with hospitals, and with pharmacies.
• Payment: we use and share health information to bill and obtain payment from your health plan, including prior authorization requests.
• Health care operations: we use health information for quality review, training, credentialing and practice administration.
• Required disclosures: immunizations are reported to the Iowa Immunization Registry Information System, and certain communicable diseases are reportable to public health under Iowa law.
• Suspected abuse or neglect: Iowa law makes health care providers mandatory reporters. We will report where we are required to, and we will tell you when we are permitted to.

Your rights over your child's health information

Under HIPAA and Iowa law you may:

• Inspect and obtain a copy of your child's medical record, in paper or electronic form.
• Ask us to correct information you believe is incorrect or incomplete.
• Request confidential communications, such as asking us to call a specific number or write to a specific address.
• Ask us to restrict certain uses and disclosures, including a restriction on disclosure to a health plan for care you paid for in full yourself.
• Receive an accounting of certain disclosures we have made in the previous six years.
• Receive a paper copy of this notice, even if you agreed to receive it electronically.
• File a complaint with our Privacy Officer or with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.

Adolescent confidentiality

From age 12, parts of your child's record become confidential to them under Iowa and federal law, particularly around reproductive health, sexually transmitted infection treatment, substance use treatment and mental health care. From that age we spend part of every well visit alone with your child and explain to you both what stays private and what does not.

Confidentiality is never absolute. If a young person tells us they intend to harm themselves or someone else, or discloses abuse, we act on it and we involve you. Safety overrides privacy every time.

The patient portal and electronic communication

Parents and guardians hold the portal account for children under 12. At 12, the adolescent portion of the record becomes restricted in line with the rules above, and teenagers 12 and over can be given their own login. Portal messages are part of the medical record.

The portal is not a route for an urgent problem. For anything acute, call us. If this is a medical emergency, call 911.

We may update this policy from time to time. The date at the top of this page shows when it last changed.
